Non-invasive sealing with the use of glass ionomer sealants

Indications:

• eruption of temporary molars in early childhood caries;

 • eruption of permanent molars and premolars with low ILM fissures.

The peculiarities of the technique are that when working with glass ionomer sealants, the etching stage with the use of phosphoric acid is not carried out. Before applying the glass ionomer sealant, it is possible to use dentin conditioners (under good working conditions, since we are talking about working with young children).

 

Non-invasive sealing with the use of compomeric sealants

Indications:

• eruption of permanent molars and premolars with medium and low IUM fissures.

Features of the technology. All compomeric sealants are used with self-etching adhesive systems. After their application, they do not need to be washed off and dried. Adhesive systems are applied to the cleaned chewing surface of the tooth, then a sealant is applied, then polymerization is carried out.

 

Invasive sealing of fissures using composite sealants

Indications:

• initial fissure caries;

 • hard-to-reach narrow and deep fissures in the teeth at the stage of enamel maturation.

Methodology of conducting:

1. Clean the tooth with a mechanical brush and polishing paste. More effective cleaning of the chewing surface is carried out using sandblasting machines.

2. The entrance to the fissure is opened with the help of diamond spear-shaped bores (fissurotomes) by removing hard tissues in the area of the walls of the fissures. After grinding, the bottom and walls of the fissure should be accessible for inspection, and hard tissues should be dense when probing

3. Isolate the tooth from saliva and etch it with 30-37% orthophosphoric acid solution for 15-30 seconds.

4. Remove the etching agent with a jet of water for 30 seconds. Dried.

5. An adhesive system is applied and polymerized.

6. Apply sealant using a probe, cannula. The material is carefully distributed to avoid the formation of pores.

7. Photopolymerize for 20-40 seconds.

8. Remove excess, control occlusion; if correction of the sealant is necessary, use diamond burs, finiers and polishes.

9. Local fluoridation is carried out.

 

Preventive filling (conservative adhesive restoration)

Indications:

 • small carious cavities (no more than 2 mm in diameter) localized on the chewing surface of molars and premolars.

 

 Preventive filling includes preparation on the chewing surface of molars and premolars of a small carious cavity (up to 2 mm in diameter), its direct filling with various filling materials and subsequent application of sealant to the surface of the seal and adjacent healthy pits and fissures. The preventive coating provides additional micromechanical fixation of the filling mass on the tooth surface, significantly reduces the risk of secondary caries and protects healthy pits and fissures from caries.

 

Methodology of conducting

1. Thoroughly clean the chewing surface with abrasive pastes and diagnose the condition of fissures.      

2. The carious cavity is dissected with fissure and small spherical bores, only demineralized tissues are removed.

3. Isolate the tooth from the oral cavity and etch with orthophosphoric acid for 15-30 seconds.

4. Remove the etching agent with a water jet for 30 seconds. Dry the tooth.

5. Apply an adhesive system, photopolymerize

6. A composite is applied (the filling material is placed just below the edges of the carious cavity), photopolymerized.

7. A layer of composite sealant is applied to the area of the seal and adjacent fissures and photopolymerized.

8. Correct occlusion, polish
